The main problem is the video typically runs at a higher bit rate than the audio, and will always be more information to process. Depending on who releases it and therefore how it's rendered one of two things happen; Imagine the audio stream in the file as a fit 120lb marathon runner, he has no problem jogging along endlessly. Now imagine the video stream in the file as well, me, a 250lb guy who can easily bench the marathon runner but get's winded a block down the street. What usually happens is the audio stream will out run the video stream and after a minute into the file they can be separated by as much as 10sec, making the video impossible to watch. The other thing that happens is the audio actually stops every few milliseconds so that the video can catch up making the sound choppy as in Natsume Yuujin Chou.
