I use VirtualDub with the DirectShow plugin to extract the video frames and convert them to images and then Photoshop CS5 to make the actual gif. Ga-rei uses a similar method, Joss uses a different one.

Photoshop CS5 isn't for the faint of heart but if you're interested, there are tons of guides out there, including some on Youtube like this one.
