2nd question: not necessairly (some are, and some not. don't really know about the majority of them though)
1st question: i think it depends on how frequently the given manga comes out, and not really a matter of weeks (e.g. weekly, monthy, etc)

The typical release schedule for manga can be weekly, monthly or bi-monthly, (one every two months),. Weekly titles have between 15-25 pages. Monthly Titles between 35-50 pages. Bi-monthly between 30-70 These are averages as each one can have a few pages more/less.
As for adaptations from manga to anime, a lot depends on how much the anime deviates from the manga. But usually it comes out to around 1 3/4 to 2 chapters for each half hour episode.

a weekly manga magz is the one i know the fastest one, about 4 - 5 days(with/without additional assistants depends on how close the deadline and how much the work pile up) for 1 chapter(18 - 22 pages). how fast the mangaka varies on the assist and him/herself, maybe can be faster than 4 days(4 - 5 days is the given time to finish the work)
a monthly one - this i dont know but usually 30 - 40 pages or more, usually assisted when only needed,
just for a note, i page manga BW consist rough sketch, pencil, ink, cleaning, screentone, finishing touch.
2 chaps = 1 ep anime? who knows? depends on the story board arranged by the person in charge.
if 1 chapters full/packed with many things like 4 koma manga, 1 chapt could be 1 ep anime, vice versa
all of your questions, will say it depends on how long the story it would take.
on your first question, I would say It depends on what is the plot of the story and how many people who are drawing for that chapter. Usually on the first chapter of the story reaching from 20-60 pages, because of the introduction of the characters and their backgrounds. If the story was in progress it does from 15-40 pages per chapter, its more on 1-3 weeks to make a single chapter.
On your second question, this also varies, depending also on the story board, if that approves, then its showtime.
