The opening for Cowboy Bebop, easily. It also has my favorite ending theme...really, all the music in the anime is just fantastic. I just love the visuals on the intro as well, though, with the James Bond style motions and color palettes. It's great.

for both song and video, it would have to be d gray man op 4, gekidou by uverworld.
oh, and i also like the op of rideback, awesome video, awesome song by mell.
as for the ed, it would have to be soul eater ed 2 style by kana nishino and hayate no gotoku season 2 ed by shizuka itou^^
